To schedule an appointment, please call .
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
Co-payments, co-insurances and deductibles are due at the time of visit. We also accept the following credit cards:

Office Hours
To schedule an appointment, please contact our office. Dr. Walden is also on call. If there is an urgent need to reach Dr. Walden outside of normal office hours, please call the office and the doctor will be notified immediately.

Insurance
We participate in Medicare, most other insurance plans, and many managed care and HMO's. Always bring your insurance card(s) for each appointment. Always keep us informed of any changes to your phone number, address or insurance. Covered procedures will vary with each insurance company -- our office staff will assist you in determining eligibility.

Updating Records
It is very important that your doctor have your up-to-date address and phone number. In case of an emergency we must be able to contact you. Please make sure the receptionist is aware of any changes. Also, please make sure we have both work and home phone numbers and another emergency contact number if possible.
