The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit.

Office Hours
For information or to schedule an appointment, please call us at 512-640-8111.
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.
Dr. Walden are also on call. If there is an urgent need to reach one of your providers outside of normal office hours, please call the office and our answering service will notify the doctor on call immediately.
Monday-Friday: (9:00am – 5:00pm)
Consultation Fee
There is a $100 fee when scheduling a consultation with Dr. Walden. Payment can be made by cash or credit card. Contact our staff for more information.
Cancellation Policy
Our office does require a credit card on file when booking any surgical/non-surgical consultations appointment for our cancellation policy.
If you need to cancel your appointment, we respectfully request at least 24 hours notice. Our Policy: Any cancellation or reschedule made less than 24 hours or no show will result in a cancellation fee. Our cancellation fee of $100 will be charged to the card on file.
Insurance
Dr. Walden's office does not participate in any health insurance plans. Office policy requires that all payments to be made prior to the time of service.

Updating Records
It is very important that your doctor has your up-to-date address and phone number. In case of an emergency we must be able to contact you. Please make sure the receptionist is aware of any changes. Also, please make sure we have both work and home phone numbers and another emergency contact number if possible.
